Who can refer
We welcome clinically appropriate referrals from physicians, advanced practice clinicians, therapists, case managers and other members of a patient’s care team. Attorneys and claims professionals may also contact the clinic regarding evaluation logistics for appropriate cases.

- Diagnosis and reason for referral
- Relevant treatment history
- Current medication list
- Pertinent psychiatric, neurologic and cardiovascular history
- Recent specialist notes, imaging or testing when relevant
Behavioral health coordination
For psychiatric indications, ongoing mental health care is often an important part of the treatment plan. Ketamine should not be presented as a replacement for crisis care, psychotherapy, medication management, or other necessary services.
